How to capture additional info at employee level

Hello All,

We wanted to capture some extra info shown in below image (Certification details that employee has earned) against employee record.

Certification Name 1   Start Date   End Date  
Certification Name 2   Start Date   End Date  
Certification Name 3   Start Date   End Date  
Certification Name 4   Start Date   End Date  
Certification Name 5   Start Date   End Date  

 

What is the best way/approach to achieve above requirement?
