Rel13: Create Quick Action doesnt work

Hi All,

I am trying to create a Quick Action for a page entry that I created using Page Integration. I can click on create on the quick actions page, add all the details and save it.

However, when i navigate away from the page and come back to the same page under Structure > Me > Quick Actions, I dont see the quick action just created under Personal Info anymore.

Has anyone had this issue before?

Please advise.
