Getting same answer to the question for all employees check-ins template in BI Report

I am making a report to get the answers to questions of check-ins templates.I am getting same answer for all employees who are in same check-ins meeting template. I checked the query joins. i am getting same ids for joins. i am not getting whether this is join or data problem.
For example as i shown in screenshot ,Neel Dave submitted answer is ‘No Answer’.But in the query result we are getting ‘Yes’ which is wrong result.So something wrong in joins flow or in the data.
I am attaching Bi query.Please check the query.If joins are wrong please let me the correct joins.
