Getting duplicate lines of data for each DSQ added to a report

Hello,

I am working on creating an applicant report which has the answers to 3 x disqualification questions on it.  The issue I have is that when I add the 3 DSQ to the table and I view the report it is creating 3 rows of data for each candidate (1 row for each question). Below is a cut of the data for one candidate where I have removed any sensitive data but shows the 3 DSQ and how it is creating extra rows.
