Taxable Journal. Getting error in each line

While trying to enter taxable journal getting error in each lines

Hi,

 

General Accounting > Journals>  Taxable Transactions > Create Taxable Transactions in spreadsheet.

and entered values  , while trying to upload it , it got failed.

Dont know when credit ,debit correct accounts are entered then why it is asking for that

While i am trying to enter Taxable Journal, getting below errors while uploading .

Please find attached for step wise screenshot and advise if i am entering wrong values
