Deployed DFFs not showing on req form

Created and deployed DFFs are not showing

Content

I created several DFFs and deployed them.  They showed up on the details page.  I then published a sandbox (wrong order I know) and they disappeared.  I then deployed them again and didn't show.  Deleted them and recreated, deployed and won't show.  Anyone have any thoughts as to what I am missing?
