e-Signature Date From Matched Application

I am trying to pull the e-Signature date for a candidate from a matched application. (So, the candidate e-Signed a previous application and then was matched to a similar REQ). I want this all on one row. How do I do that?

I am doing this in Recruiting (Legacy). 

In my example image, I have Candidate ID, REQ Number, e-Signature Date (matched to REQ), Application Matched From Number, e-Signature Date (of Application Matched From REQ).
