BI Publisher Plug-In for MS Word - Not Working

For some reason, I'm unable to setup the BI Publisher plug-in for MS Word.   We've followed the instructions located here:

While I can download the file and run the install for BI PUblisher, looks like all is well.  However, on one machine I can't start the server at all.  On my second machine, I can start the server and open word, but no menu for BI Publisher appears.  I've googled and followed instructions on how to fix it but no luck.
