Find the field with the performance score of the nine box

Good afternoon,

It is my first post so please forgive me if i make a mistake 

I am working on a report about people review and we want to display the employee's box (future star, top talent, under-performer, etc...)

But i think this information is not in the tables or views. So I have to find the scores of performance and potential to do condition and display the employee's box. (If potential score = 3 and performance score = 2 the employee's box = Future Star.

I have already find the potential score in the field "POTENTIAL_SCORE" in the table "HRT_CAREER_POTENTIAL_ITEMS_V "
