Auto Rejected Requisitions - "Approvals encountered an error."

Hi - we're using Oracle Cloud Application 19B (11.13.19.04.0).

Issue Background

We are routinely experiencing issues where users submit requisitions for approval, but the REQ is auto Rejected.

When we view the Document History, we see that the Action Performed = Rejected and Additional Information = "Approvals encountered an error." (Screenshot 1)

We are having problems figuring out the cause of these issues.

Manage Approvals

We have found via testing that if, when raising the REQ, if the user clicks on "Manage Approvals" then if there is a problem then an error appears which can sometimes help with diagnosing the issue.
