Trying to pull Comments from Mid-Year Performance document into a report

When Employee/Manager Comments fields are added to performance report, errors out

I have a request to pull a report of all Goals and include the Employee and Manager Comments that were entered in the Mid-Year Performance Document.  As soon as a Comments field is added, I get the ODBC error.  Measurement fields are pulled in from each Subject Area.  What could be causing this error?
