Element Adjustment

Voluntary benefit amount adjusted

We have an employee whose voluntary benefit amount changed on the last payroll. Their election at the beginning of the year has never changed and there was no life event that altered it either, however looking at the deduction report and the payroll activity report, the employee's balance value for the last pay check decreased significantly. What could have possibly caused this? If there was an adjustment made to the element, where can I search to find that?
