While creating a questionnaire, cannot go past the "basic information" stage in the creation process.

Hi,

While creating a questionnaire I get the below error message.

The questionnaire is being made from a template that has been successfully created and appears when creating the questionnaire, and changing the "Questionnaire Code" on the same page that the error is on also yields no different response.

What is going on?
