Using the same candidate field for multiple rows on an application

Any suggestions on how to use the same candidate field for multiple entries on an application?

I'm trying to replicate the table below, that's currently in a paper application, on a user-defined form. I created user-defined fields for Date, Nature of Accident, etc. but I need to make the fields available four times so they can add up to four entries. However, the same field can't be used more than once in a form.

The only way I could think of is creating the same field four times. I did notice on the field editor attributes there is "Num. of lines." Is that relevant to this?
