Questionnaire Reporting Issue

Report using questionnaire section not returning results as expectedContent
One of our business units is using a questionnaire section in their performance review to complete quarterly check ins. I built a custom report on just the questionnaire section so the business could make sure employees are completing their check ins as expected. However, the report is not returning consistent results, and I'm having trouble narrowing down the issue. As example, when the report was run for the entire population, one employee was shown to be missing their Q2 check in.
