APEX not responding on OCI - "Service Unavailable (oracle.ucp.UniversalConnectionPoolException)


My APEX service is returning 503 Service Unavailable, reporting the following error:

The connection pool named: |ocid1.tenancy.oc1..aaaaaaaa3ulfbfoabekoo46h5lmbfhnxgk6oell2djfefvvuehkmsk2zeoya-dbasg20190923|re| is not correctly configured, due to the following error(s): Exception occurred while getting connection: oracle.ucp.UniversalConnectionPoolException: All connections in the Universal Connection Pool are in use

This has been going on for more than a few weeks; initially the error was intermittent but now it occurs every time.

I've tried stopping and restarting the autonomous database but the problem persists.
