Need Help - have user that is employee but no Smart Org User - Internal Cand there

We are integrating with HCM and there is no error on our employee net-change for this person.  This person has an internal candidate record but there is no Smart Org user account (still baffled by the data structure and schema - why 2 different things ...)

How does one fix this issue?  Trying to enter a user account but get email in use error .... 

Anyone have an idea?
