Attachments not moving to Document Records

Hi All,

We have a custom phase in our CSP between offer and HR phase.This phase is primarily to store the information related to Medical check ups and Reference information once the candidate has accepted the offer.This information is primarily in form of attachments.When I attach these on the Attachments tab on Candidate application they do not appear on Document Records on person record once the candidate is converted to a worker.I only see the Job offer appearing under Document Records but not any other attachments.
