Unable to connect to Oracle using HSDL

When running a HSDL template unable to connect

I am trying to run a spreadsheet template.  When i click on run spreadsheet template, the spreadsheet opens; however, the screen asking if i want to connect never pops up and the template is blank.

Besides pop up blockers (already checked) what would prevent me from connecting to Oracle?
