Would like a free form text box for Agency Source Type

Need candidates to be able to type text for Agency Source TypeContent
We would like our candidates to enter the agency in a mandatory text box when they choose "Agency" as a Source Type. See screen shot below. Where *Agency is, we would like the candidate to type free form text and have it be mandatory. Similar to the "Other" source type. Is this possible?
