Need help troubleshooting a few things on a filled requisition report

Hello! Looking for some help troubleshooting the attached filled requisition report. We're stuck on a few things:

1) Time to fill is not pulling consistently for each requisition

2) Why are only certain candidates showing up on this report? It doesn't look like we have any filters set based on candidates (we do eventually want to filter down to just hired candidates but for now curious as to why it's limited)

3) We're struggling to format the final columns which we have in a pivot table now. The goal end result would be to have a column for each prescreening question we want to report on, but we can't seem to accomplish this without duplicating columns.
