Need help to create an Eligibility profile

Hi All,
I am trying to create an Eligibility profile to assign goal plan. And below is the condition
Employee Hire Date less than or equal to 6 months AND Hired between (Jan-June) = Probationary Goal Plan
Employee Hire Date greater than or equal to 6 months OR Hired between (Jul-Dec)= Annual Goal Plan
Please suggest me how to achieve this requirement.
