Department Changes - BI Report

Shows department changes for an associate for the current yearContent
Hello,
We are looking for a BI Report that shows department changes for the current year. Is there a subject area in BI reports without using a Data Model that can pull previous and new department? We currently have a Data Model set up for this but it would be ideal to have a BI Report to add to our Turnover Dashboard.
What we are looking for on the report is to show:
- Person Number
- Name
- Effective Date of the change
- Action
- Action Reason
- Previous Department
- New Department
