Voluntary Self-Identification of a Disability - New Form for 8/4

Hi,
I activated the new disability form in our staging environment and when I tested it, it works fine on a desktop but I'm having issues on a mobile device. The form appears very small and makes it difficult, if at all possible, for a candidate to enter information into the text fields (name and date) and select an answer.
This form is part of our first pass application and we're worried this will affect our applicant flow if they get frustrated that they can't get past this page to submit their application.
We submitted a SR to Oracle who indicated the issue is with the branding file. We've reached out to our internal marketing team but we are having some struggles on getting this to work.
