    Unable to add User defined fields to User defined Forms

    We are trying to edit the User defined Forms. We noticed that 2 of the fields that were added to the user defined forms for requistion are not available under the UDF defined under requisition. 

    When we tried to create a new form and add these 2 fields to that form, we are not able to find the fields under the field chooser.

     

    Is there a way we can add these 2 fields to the new User defined forms
