Prevent the hiring managers approving offers on behalf of candidates and extending offers prior to approval

We have the following options set but still hiring managers are able to approve offers on behalf of candidates and they are able to extend offer before the approval.   

Our requirement is to prevent the hiring managers approve the offers; Hiring managers should not be able to extend the offer prior to approval. We just upgraded to fluid.
