Extend Offer button not going anywhere

When clicking on 'extend offer' nothing happens

When users are completing the offer grid, a letter has been generated and they try to extend the offer via e-offer, the 'extend offer' button doesn't go anywhere and the offer letter cannot be sent. Has anyone come accross this issue before? Its occuring in both Fluid and Legacy pages.
