How to configure the application process so that the candidate has to enter their Referrer Details i

Hi Experts
Currently we have an option as part of our application process for the candidate to select if they are a referral by an existing employee within our business. However, I was wondering if it were possible to have it set up that if they select that they are, more fields will appear where they can enter in the details of their referrer (eg. name, position etc).
I have looked in the Configuration module > Career Section and we appear to have a User defined form with these details - but currently this is not appearing in the career section when the candidate is applying.
