Resignation Submitted date

Hi All, 

 

We had a predefined report which use to give Resignation Submission date along with the Employee last working date.

 

Few days back the report stopped working and we had to take off the resignation submission date to extract the data from OTBI. 

Can someone please help on any report or something which can help us to extract the both the data points. 

 

Kindly do let me know if I need to any more clarification to this.
