How to use E-signature with PDF

Content

Looking to have applicant who is completing onboarding task fill out a PDF and then have it E-Signed.  I am not able to see how to create a task with a PDF with an e-signature.  I have tried to use a PDF form but the output to the applicant is all messed up.

 

Any advise.
