When Extending an offer, the message that shows says "The Job Offer is being extended to recipient John Doe"

The state stays in Approved an no offer goes out.  Any thoughts as to why this message instead of  the typical "The Job Offer has been extended to John Doe"

Offer is complete and offer letter is attached.
