We receive the following error when trying to add a question to a req.  The req is open, unposted and in draft mode.

The description of possible answers must be provided for all activated languages.  The following translations are missing: English (Base)

 

In the past this error happened when trying to add multiple questions at the same time but this is happening when only one question is added.
