Pull Succession Plan Name with Talent Profiles.

I have created a report to pull latest Performance, Potential, Risk of Loss And Impact of Loss from Talent Profiles.
Is there a way to add Succession Plan name to this report on a new column if the employee is in a succession plan?
I am unable to Join Person Profile and Plan Candidates Real Time subject areas. catalog file attached for reference
