We want to display a question on the job application, that reads, How did you hear about us?
The applicant can select from 5 values in the lov's, 2 of the values requires the applicant to enter a response. For each value that requires a typed response, a textbox should display.
Example: How did you hear about this job?
Values in a list: Indeed when this value is selected a textbox should not display.
Monster when this value is selected a textbox should not display.
Friend (enter their name) - when this value is selected a textbox should display.
