JBO-27101 Dead Entity Error When Creating Expense Item from Expenses module

Sporadically for a few users, when trying to create a new Expense Item, they are receiving a JBO-27101 Dead Entity Error message and cannot proceed with creating the item.

JBO-27101: Attempt to access a dead entity in ExpenseEO, key=oracle.jbo.Key[****]

I am curious if any others are experiencing this in their Production pod. If yes, what were your problem solving tactics? Thanks!
