Offer to candidate made, then candidate is automatically rejected

We have a hiring manager who extended an offer to a candidate. The offer immediately changed to withdrawn by employer and the offer was rejected, sending the rejection email to the candidate. The transaction console show's that the hiring manager withdrew the offer, but she didn't. Not sure why this happened, anyone have any ideas? Our Recruiter was able to re-create the offer which worked fine - not the idea work around.
