Prescreening Questionnaire Applicant Responses not working as expected

There are multiple issues we are noticing we the prescreening questionnaire. The first issue is that the conditional question response is not visible from the Questions tab in Hiring. However, I can see responses from the backend reporting (see image 1). In imagine one, this candidate selected a response which prompted a second response from a set list of values to choose from. They did make that selection (Glassdoor), but this is not captured on the screen, but it is on the backend reporting.
The second issue is that values that are entered on the front end are not visible on the backend. For example, with image 1, this candidate also provided salary expectation, but this value is not visible in the report.
