Offer Approval notification not getting triggered

We are trying to send out automated notification once an Offer is approved.

There is a seeded Message template that does the work, but is not getting triggered once the checkbox to send acknoledgement is checked.

Can someone please help me with the navigation to where the configuration is to configure the Offer Approval Request form. Is there a way to default the checkbox to get auto selected. Attached is the screenshot for reference.

Or is there might be help me in figuring out why the notification is not getting triggered when offer is approved.
