Checklist Application Tasks have changed

Previously configured application tasks have been wiped out


Hi, we configured checklists a few months ago, some of our tasks make use of the "Application Task" Task Type. Namely we were using the Application Tasks "Personal Details", "Family and Emergency Contacts" and "Document Records". Upon going into my templates to make updates I have noticed that the Application Task is now blank for the previously configured tasks. Both "Personal Details" and "Family and Emergency Contacts" no longer exist in the list of Application Tasks. Also when I go to add back in the "Document Records" task the application is making it blank again after saving.
