Candidates "bypassing" the address fields on application

We have, in the past 2 weeks, had a report of candidates (we are finding this DURING the convert phase where you cannot edit this info) not having their full address on their record. They are applying themselves, and somehow able to get into the system. We are working on a solution/why for this issue, but I am checking to see if anyone else has had this happen. 

Our address block is required on our application. City, State, Zip, County, Country.  We also have tried to recreate this issue ourselves and cannot seem to. When we look at a candidate that made it through onto a req it shows 2 lines of the street address and only the country.
