Help with OBI: How to capture Offer Approvers, Decision and Date

Hi all, 

I need to create a report in OBI to capture our offers and the approval path (including decision and date). From what I can see in OBI, there are not separate approver fields for the offer like there are for the requisition. Therefore, when I try to create the report now, each approvers details appear on their own row causing multiple rows per offer. I would like the report to be in the format seen in the screenshot, where there is 1 row for the offer and then different columns to show the approver, their decision and the decision date.
