Unable to display the Line Manager name in Alert Notification

Hi,

Can someone please help on how to display the Line Manager name and Email Address in the Alert Notification ?

I have initially tried to display in the tabular format (below code) which is working fine. But the client wants to trigger individual email notification for each termination. Hence the data needs to be displayed in rows instead of tabular format.
