View issues with Voluntary Self-Identification of Disability Form

Employees can no longer see the form in Talent Center

When a new hire as been assigned the Voluntary Self-Identification of Disability Form, they cannot see the contents of the form in the talent center. The only thing visible are the fillable fields.  Is there something going on where this form is no longer useable?
