HSDL Error when trying to add "GL Cost Center Information" against Department

I'm trying to use a HSDL Spreadsheet Loader to load Cost Centres against Departments.
To do that, I went to Setup and Maintenance > Manage Departments.
I created a new Department called "XXJR Dept 000 - 0999".
Then clicked on "Manage in Spreadsheet".
From there, I went through the steps shown in "Screenshots 1" screenshot attached to this post, but I end up with this error:
The line for component OrgInformation with instruction MERGE doesn't include values that define a unique reference to the record. Include values for at least one of the supported keys.
