Why did a Reconciliation Withdraw occur?

Summary:

We created a voluntary assignment for learners in an offering. When the instructor logged in to mark the learners complete in the ILT, they were already withdrawn, with a Withdraw Type of Reconciliation Withdraw

The assignment did not have a completion date and the withdraw occured before the offering ended and before the activities ended. What would cause this Reconciliation Withdraw and how can we stop it? Also, is there a way to change a student's withdraw status back to Active or In Progress?
