how to view e-signature report after task complete using per_checklist_tasks columns

Using Doc ID 2611795.1 as a reference, we realized that we used screen fields to populate the report. We tried to use signature columns from per_checklist_Tasks to use in it, but we noticed that these columns are only written after the report is placed in the doc of records. Does anyone else realized this?  

the document says "filled or unfilled , report show the fields P_SIGNER_NAME, P_SIGN_DATE, etc" . i wanna to use per_checklist_tasks.signer_name , sign_Date, etc instead, but when try to do this, the document save blank signature in doc of records, but with signature filled in per_checklist_Task correctly.
