Verification notification email sent twice

Hi,
I have noticed that sometimes, when candidates click on the button of the Request information email that redirects them to the questionnaire, they receive the verification notification email with the access code twice, which is confusing since it is not clear which is the one to use.
Any idea on why the email with the code is sent twice? It's strange since it seems random.
