Termination Report needed (workflow initiated date vs the actual termination date)

Summary:

We are in need of a report which will help us catch "late terminations". Report should have a date in which the workflow was initiated, and then the termination date itself. This way we can tell if a manger submitted a termination before the actual termination date.
