OBI Report - Files, Forms, and Fields

Good Afternoon,

We are attempting to pull an OBI report of all custom and standard fields we currently have in use today. We would also need to pull the User Defined Form those fields are binded to in order to confirm if they are currently being used today, compared to those that are not in use.

How can we pull this information through OBI? Or is there another method?
