How do you display Definition Help Text or Instruction Help Text that is setup on checklist DFF?

I have created a configurable form via the checklist DFF. For each context sensitive segment I have added Instruction and definition help text to be displayed on the form. However this text is not visible. Is there other setup required to enable the text on the form?
